Abstract

A clinico-pathological study was done in 29 cases of Wilms’ tumor with children whose ages ranged between 7 months to 9 years in order to evaluate the factors related in the prognosis. From the clinical standpoint the following results were obtained:

Prompt nephrectomy followed by post-operative radiation therapy and/or Actinomycin D administration was the type of treatment and the rate of survival was 44%.

The rate of survival below the age of 1 year was 100%, 1-2 yrs 50% and 2-9 yrs 23%.

A) Pathological findings seen as a poor prognostic sign included:

The presence of metastasis

Tumor invasion in the vessels,

Tears in the capsule

The presence of undifferentiated epithelial cells.THE PROGNOSIS OF WILMS’ TUMOR

B) Pathological findings seen as a good prognostic sign: Presence of glomerular structures.

C) No relation could be found as far as prognosis was concerned regarding the following factors:

The weight of the tumor

Tumor invasion in the renal pelves and the ureter

The area of the kidney involved by the tumor

The presence of undifferentiated mesenchymal cell sheets

Tubular structures and differentiated skeletal muscle.

D) Doubtful findings with regard to any prognostic significance: Presence of fat, cartilage and bone in the tumor.
